Infertility ids defined as the inability to achieve pregnancy after one year of regular, unprotected intercourse. Traditionally, infertility was thought to be due to female factors alone, but it is now better understood that infertility is not just a female problem. Evaluation and treatment may be initiated soon after one yearof unprotected intercourse in female under the age of 35 and after six months if the female partner is older than 35 years. Major causes of infertility in males include male factors such as Azoospermia (seminal fluid without sperms), reduced number and motility of sperm cells and their morphological defects. Sperm DNA fragmentation is completely independent of the count, motility or in morphology and where the routine semen analysis shows normal parameters, an estimation of sperm DNA integrity may predict male infertility. Intrauterine insemination (IUI)Intrauterine insemination or artificial insemination is a fertility treatment that put washed and concentrated sperms directly inside the uterus around the time of ovulation; to facilitate fertilization. The next step is to watch for signs and symptoms of pregnancy. IUI is used most often in couples who have unexplained infertility, sub fertility, cervical factor infertility and ovulatory factor infertility. The IUI procedure takes only few minutes and involves minimal discomfort. It can be less expensive than other type of fertility treatments.
In vitro fertilisation (IVF) and Intracytoplasmic Sperm Injection (ICSI )Invitro fertilization or IVF is a method of assisted reproduction, in which a man’s sperm and a woman’s egg are combined in a laboratory dish. Resulting embryos may be transferred into the woman’s uterus. IVF can be used to treat infertility inpatients with blocked or damaged fallopian tubes, male factor infertility, women ovulation disorders, premature ovarian failure, uterine fibroids and in the case of unexplained infertility. Ovarian stimulation, egg retrieval, fertilization, embryo culture and embryo transfer are the basic steps in an IVF treatment cycle. Intracytoplasmic sperm injection is an assisted reproductive method where a single sperm is picked up with a needle and injected directly into oocytes. PESA/TESA are the two techniques which revolutionized treatment of male infertility in the past decade. PESA or percutaneous aspiration of sperm retrieval and TESA or Testicular sperm extraction does not require surgical incision and can be performed under sedation or local anesthesia. The retrieved sperms cells are used for ICSI. Men with azoospermia(both obstructive and non –obstructive) benefit form PESA/TESA.
Cryopreservation of Gametes and Embryo
Cryopreservation-the ability to freeze and thaw with the retention of viability-provides flexibility in human infertility treatment because frozen tissues can be stored in liquid nitrogen at -1960 C for a very long period of time. Third party reproduction
Bliss health care offers sperm donation, egg donation, and embryo donation programmes as per ICMR norms. Donor sperms are obtained from sperm banks and egg donors are obtained from ART banks. Donors are screened for various parameters and will be anonymous at all times. These services we are only offering at our Palakkad center, that too in extremely indicated cases. At Kottayam center we are following 100 % non-donor policy.
